Resumo Introdução A apneia obstrutiva do sono é consequência do colapso das vias aéreas superiores Qualquer região de obstrução nas vias aéreas superiores pode contribuir para o colapso da faringe. A obesidade e os distúrbios relacionados à obesidade desempenham um papel importante na apneia obstrutiva do sono e sua relação com o aumento da resistência das vias aéreas superiores. Objetivo Avaliar a relação entre a obesidade e as propriedades da cavidade nasal em pacientes com apneia obstrutiva do sono. Método O estudo foi feito retrospectivamente através da revisão de prontuários médicos de pacientes adultos. O instrumento de avaliação NOSE, do inglês nasal obstruction symptom evaluation, foi usado para avaliar a obstrução nasal. Os distúrbios respiratórios do sono foram avaliados através de exames polissonográficos. O volume nasal foi obtido por tomografia computadorizada e a reconstrução volumétrica das vias aéreas nasais. As alterações anatômicas nasais foram avaliadas por endoscopia nasal. Resultados A análise dos prontuários de 83 pacientes, entre os quais 54 eram do sexo masculino e 29 do feminino, encontrou um índice de massa corporal médio de 28,69 kg/m2. Os grupos obeso e não obeso foram determinados com o ponto de corte de 30 kg/m2. Na comparação entre os grupos, o grupo obeso apresentou correlação positiva e significante com o índice de apneia/hipopneia (p = 0,02), instrumento NOSE (p = 0,033) e hipertrofia da concha inferior (p = 0,036), com uma odds ratio de 1,983 (IC95%: 1,048 a 3,753). A avaliação do desvio do septo nasal (p = 0,126) e do volume das vias aéreas nasais (p = 0,177) não mostrou resultados significantes. Conclusão A obesidade correlacionou-se significantemente com a obstrução nasal subjetiva pela escala NOSE e hipertrofia de concha inferior em pacientes com apneia obstrutiva do sono. Não houve correlação com a avaliação do volume nasal. Nível de evidência 3b. Estudo de caso-controle individual.

Abstract Introduction Obstructive sleep apnea is a consequence of upper airway collapse. Any obstructive sector in the upper airway can contribute to pharyngeal collapse. Obesity and obesity-related disorders play an important role in obstructive sleep apnea and its relationship with increased upper airway resistance. Objective This study was designed to evaluate the relationship between obesity and properties of the nasal cavity in patients with obstructive sleep apnea. Methods The study was conducted retrospectively by review of medical records of adult patients. The nasal obstruction symptom evaluation, NOSE instrument, was used to measure nasal obstruction. Sleep breathing disorders were evaluated by polysomnography exams. Nasal volume was obtained by computed tomography scans and volumetric reconstruction of nasal airway. Nasal anatomic alterations were assessed by nasal endoscopy. Results Analysis of 83 patient records, among whom 54 were male and 29 females, found the mean body mass index of 28.69 kg/m2. Obese and non-obese groups were determined by using cut-off 30 kg/m2. In the comparison between groups, the obese group had a positive and significant correlation with apnea/hypopnea index (p= 0.02), NOSE instrument (p= 0.033) and inferior turbinate hypertrophy (p= 0.036), with odds ratio 1.983 (95% IC 1.048 − 3.753). nasal septum deviation (p= 0.126) and nasal airway volume evaluation (p= 0.177) showed no significant results. Conclusion Obesity was significantly correlated with subjective nasal obstruction, NOSE scale, and inferior turbinate hypertrophy in patients with obstructive sleep apnea. RESUMO OBJETIVO Conduzir uma análise de custo-efetividade das alternativas para tratamento reabilitador do edentulismo mandibular no contexto do Sistema Único de Saúde (prótese total implanto-suportada e prótese total convencional). MÉTODOS Foi desenvolvido um modelo de Markov para captar os resultados clínicos e econômicos de longo prazo. A população do modelo consistiu em uma coorte hipotética de 1.000.000 pacientes, com 55 anos, desdentados totais mandibulares e sem contraindicações médicas para a realização de procedimentos cirúrgicos. A perspectiva de análise adotada foi a do Sistema Único de Saúde. Com base no modelo proposto, calculamos o custo (em reais) e a efetividade, medida pelo ano de prótese ajustado à qualidade (QAPY). O horizonte temporal da análise foi de 20 anos. RESULTADOS Considerando o desconto de 5% nos custos e efeitos, a razão de custo-efetividade incremental da prótese total implanto-suportada em relação à prótese total convencional (R$ 464,22/QAPY) foi menor que o limiar de disposição a pagar adotado no modelo (R$ 3.050,00/QAPY). CONCLUSÕES Os resultados desta análise econômica mostraram que a reabilitação de edêntulos mandibulares por meio da prótese total implanto-suportada é muito custo-efetiva em comparação à prótese total convencional, de acordo com os limites de custo-efetividade empregados.

METHODS A Markov model was developed to capture long-term clinical and economic outcomes. The model’s population was comprised of a hypothetical cohort of 1,000,000 patients, aged 55 years, with total mandibular edentulism and without medical contraindications for performing surgical procedures. The adopted analysis perspective was that of the Brazilian Unified Health System. Based on the proposed model, we calculated cost – in BRL, and effectiveness – measured by quality-adjusted prosthesis year (QAPY). The time horizon of the analysis was 20 years. RESULTS Considering a 5% discount in costs and effects, the incremental cost-effectiveness ratio of implant-supported total dental prostheses compared to conventional total dental prosthesis (BRL 464.22/QAPY) was lower than the willingness to pay threshold adopted in the model (BRL 3,050.00/QAPY). CONCLUSIONS The results of this economic analysis showed that the rehabilitation of mandibular edentulous patients by implant-supported total prosthesis is very cost-effective when compared to conventional complete prosthesis, considering the cost-effectiveness limits employed.

Resumo Introdução A Apneia Obstrutiva do Sono ocorre por colapso recorrente das vias aéreas superiores durante o sono, resultando em redução total (apneia) ou parcial (hipopneia) do fluxo aéreo, tendo relação estreita com alterações nas vias aéreas superiores. A TC de feixe cônico permite a análise da via aérea superior e seu volume através da reconstrução tridimensional. Objetivo Avaliar uma possível correlação entre o volume da via aérea superior e a gravidade da apneia obstrutiva do sono. Método Realizou-se um estudo retrospectivo, com revisão de dados polissonográficos e registros de TC de feixe cônico de 29 pacientes (13 do sexo masculino e 16 do sexo feminino). A correlação entre o volume total da nasofaringe, a orofaringe e a faringe superior com o IAH (Índice de Apneia-Hipopneia) foi avaliada pelo coeficiente de correlação de Pearson. Resultados A divisão por gravidade da Apneia Obstrutiva do Sono foi: dez pacientes apresentaram apneia na forma severa, 7 apresentaram apneia moderada, 6 tinham a forma leve e 6 estavam saudáveis. A correlação entre a nasofaringe, a orofaringe e os volumes da faringe superior e o Índice de Apneia-Hipopneia foram respectivamente: -0,415 (p = 0,025), 0,186 (p = 0,334) e -0329 (p = 0,089). A classificação de Spearman controlada pelo Índice de Massa Corporal, idade e sexo foi: -0,206 (p = 0,304), -0,155 (p = 0,439) e 0,242 (p = 0,284). Conclusão Não há correlação entre o volume da via aérea e a apneia obstrutiva do sono, avaliada pelo índice de apneia-hipopneia e controlada pelo índice de massa corporal, idade e sexo. O volume das vias aéreas superiores como parâmetro isolado não se correlacionou com a gravidade da síndrome da apneia obstrutiva do sono, e deve ser avaliado em conjunto com outros fatores.

Abstract Introduction Obstructive sleep apnea occurs by recurrent collapse of the upper airway during sleep, resulting in total (apnea) or partial (hypopnea) reduction of the airflow and has intimate relation with changes in the upper airway. Cone Beam CT allows the analysis of the upper airway and its volume by three-dimensional reconstruction. Objective To evaluate a possible correlation between the volume of the upper airway and the severity of the obstructive sleep apnea. Methods A retrospective study was performed reviewing polysomnographic data and Cone Beam CT records of 29 patients (13 males and 16 females). The correlation between the volume of the nasopharynx, the oropharynx and the total superior pharynx with the AHI was assessed by Pearson's rank correlation coefficient. Results The obstructive sleep apnea severity division was: ten patients had severe, 7 had moderate, 6 had mild and 6 of them were healthy. The correlation between the nasopharynx, the oropharynx and the total superior pharynx volumes and the Apnea-Hypopnea-Index was respectively: −0.415 (p = 0.025), 0.186 (p = 0.334) and −0329 (p = 0.089). The Spearman's rank controlled by the Body Mass Index, the age and the gender was: −0.206 (p = 0.304), −0.155 (p = 0.439) and 0.242 (p = 0.284). Conclusion There is no correlation between the volume of the airway and the obstructive sleep apnea, assessed by Apnea-Hypopnea-Index and controlled by the Body Mass Index, the age and the gender. The volume of the upper airways as an isolated parameter did not correlate to the severity of the obstructive sleep apnea syndrome, and should be evaluated together with other factors.

ABSTRACT Introduction: Rhytidoplasty has become one of the most common aesthetic surgeries performed by plastic surgeons worldwide. Along with the increase in the number of surgeries performed, the number of procedure-related complications has also increased. In particular, nerve injuries are the major concern. By conducting a systematic review, the present study aimed to identify the main nerve structures injured during rhytidoplasty, by either the conventional or endoscopic technique. Methods: A systematic literature review was performed in the main databases currently used. Articles that met the inclusion criteria were analyzed in their entirety, and their references were checked. Finally, 20 studies were included. Results: In these 20 articles, 3,347 patients were evaluated and 142 nerve injuries found, of which 79 were of the facial nerve, 55 were of the trigeminal nerve, and eight were of the great auricular nerve. Of these, only two were definitive. The lesions were more prevalent (81%) with the video-assisted techniques than with the conventional techniques (19%). Conclusion: We found that the injuries of the temporal and buccal branches were more frequent during facelifts; and those of the great auricular nerve, during cervical rhytidoplasty. Although nerve injuries are infrequent in the literature, well-designed studies that aim to better understand these complications are lacking.

RESUMO Introdução: A ritidoplastia se tornou uma das cirurgias estéticas mais realizadas por cirurgiões plásticos ao redor do mundo. Junto com o aumento do número de cirurgias, a quantidade de complicações associadas ao procedimento também aumentou, sendo que as alterações nervosas são uma das que despertam maiores preocupações. O presente estudo visa a identificar, por meio de uma revisão sistemática, as principais estruturas nervosas lesadas durante uma ritidoplastia, tanto por técnicas convencionais como endoscópicas. Métodos: Uma revisão sistemática da literatura foi realizada nas principais bases de dados utilizadas atualmente. Artigos que preencheram os critérios de inclusão foram analisados na íntegra e suas referências, verificadas. Ao final, 20 estudos foram incluídos. Resultados: Nestes 20 artigos, no total, foram avaliados 3.347 pacientes, sendo encontradas 142 lesões nervosas: 79 do nervo facial; 55 do nervo trigêmeo, e oito do nervo auricular magno. Destas, apenas duas foram definitivas. As lesões, proporcionalmente, foram mais comuns nas técnicas videoassistidas (81%), quando comparadas com as convencionais (19%). Conclusão: Encontramos que as lesões dos ramos temporal e bucal são mais frequentes no facelift e as do nervo auricular magno, na ritidoplastia cervical. Apesar de as lesões nervosas serem pouco frequentes na literatura, faltam estudos bem desenhados que busquem conhecer melhor estas complicações.

Flexible endoscopes are fundamental in various medical specialities; in general they are heat-sensitive, semi-critical, and subject to high level disinfection. Glutaraldehyde is largely used for this purpose, due to its high compatibility and low-cost. However, its tolerance of mycobacteria and occupational toxicity lead to pressure being applied for the adoption of alternative germicides. A systematic review was undertaken aiming to seek evidence regarding the effectiveness, toxicity and potential harm caused to the endoscopes by those germicides which are alternative to glutaraldehyde and which are available on the market in Brazil. A total of 822 publications was identified in 13 electronic databases, between 2008 and 2013. Of these, 23 studies were selected, considering the best quality of evidence available. The publications point to the superiority of peracetic acid and of orthophthaldehyde regarding efficacy in high level disinfection. Only orthophthaldehyde presented an adverse event clearly related to its use. There is insufficient evidence to assert that any of these germicides has greater potential for harm to the equipment.

Fundamentales en diversas especialidades médicas, los endoscopios flexibles son, normalmente, termosensibles, semicríticos y sometidos a la desinfección de alto nivel. El glutaraldehído es muy utilizado para esa finalidad, en razón de la gran compatibilidad y bajo custo, pero la tolerancia de microbacterias y el hecho de ser o no tóxico de modo ocupacional presionan por la adopción de germicidas alternativos. Fue realizada revisión sistemática con el objetivo de buscar evidencias sobre la efectividad, toxicidad y potenciales daños causados a los endoscopios por los germicidas alternativos al glutaraldehído, disponibles en el mercado brasileño. Fueron identificadas, en 13 bases electrónicas, 822 publicaciones, entre 2008 y 2013. De estas, fueron seleccionados 23 estudios, considerando la mejor cualidad de evidencia disponible. Las publicaciones apuntaron para la superioridad del ácido peracético y del ortoftalaldehído cuanto a la eficacia en la desinfección de alto nivel. Solamente el ortoftalaldehído presentó evento adverso claramente relacionado a su utilización. No hay evidencias suficientes para afirmar que algun de estes germicidas presenta mayor potencial de daños a los equipos.

O glutaraldeído é largamente utilizado para este fim, devido à alta compatibilidade e baixo custo, porém, a tolerância de micobactérias e a toxicidade ocupacional pressionam por adoção de germicidas alternativos. Foi realizada revisão sistemática com objetivo de buscar evidências sobre a efetividade, toxicidade e potenciais danos causados aos endoscópios pelos germicidas, alternativos ao glutaraldeído, disponíveis no mercado brasileiro. Foram identificadas, em 13 bases eletrônicas, 822 publicações, entre 2008 e 2013. Destas, foram selecionados 23 estudos, considerando a melhor qualidade de evidência disponível. As publicações apontaram para a superioridade do ácido peracético e do ortoftalaldeído quanto à eficácia na desinfecção de alto nível. Somente o ortoftalaldeído apresentou evento adverso claramente relacionado à sua utilização. Não há evidências suficientes para afirmar que algum destes germicidas possui maior potencial de danos aos equipamentos.

PURPOSE: To study the repair of pericranium-cutaneous flaps fixed with suture anchored in a skull bone tunnel or N-butyl-2-cyanoacrylate adhesive in Wistar rats with emphasis on the cellular inflammatory response and the production of types I and III collagen.METHODS: The operated region in the cephalic region of Wistar rats was removed minutes before euthanasia, fixed in formalin, and subjected to histological preparation. Slides were stained with hematoxylin-eosin and Picrosirius. Standardized counts of polymorphonuclear and mononuclear cells, fibroblasts, and macrophages were performed, and the percentages of types I and III collagen were determined. Data collection occurred on days 3, 7, 14, 21, and 45 postoperatively. A value of p<0.05 was considered statistically significant.RESULTS: Quantitative analysis of the data showed more fibroblasts in the surgical adhesive group than in the nylon monofilament thread groups (p=0.0211). Qualitative analysis showed higher reactivity in the adhesive group, with a predominance of polymorphonuclear cells from days 3-45 and macrophages from days 3-7. The amount of type I collagen exceeded 80% in the treated and control groups at the end of the experiment.CONCLUSIONS: Subperiosteal detachment triggers a cellular inflammatory response that is amplified using soft tissue fixation methods. The adhesive n-butyl-2-cyanoacrylate was more reactive than the nylon monofilament thread anchored in the skull bone tunnel.

Introdução: A apneia obstrutiva do sono (AOS) é causada por episódios recorrentes de obstrução total ou parcial da via aérea superior com duração superior a 10 segundos durante o sono. Refluxo faringolaríngeo (RFL) é uma variante da doença do refluxo gastroesofágico que afeta a laringe e a faringe. Objetivos: Avaliar a influência da obesidade na relação entre RFL e AOS em pacientes com SAOS. Materiais e métodos: Estudo observacional transversal retrospectivo. Foram revisados protocolos de atendimento de pacientes com AOS que incluem questionários validados para RFL como Reflux Sympton Index (RSI) e Reflux Finding Score (RSI), nasolaringofibroscopia e polissonografia. Resultados: Cento e cinco pacientes foram divididos em grupo de obesos (39 pacientes) e não obesos (66 pacientes). Na avaliação das médias do RSI o grupo de não obesos foi semelhante entre pacientes com AOS leve (11,96) e moderada (11,43). No grupo de obesos a média do RSI foi de 6,7 em pacientes com AOS leve e de 11,53 em pacientes com AOS moderada a grave (p < 0,05). Discussão: O subgrupo de pacientes com AOS e RFL apresenta vários fatores que promovem a inflamação da via aérea superior. Pacientes com AOS devem ser pesquisados e tratados quanto a RFL, aumentando a qualidade de vida. Conclusão: O RFL e a AOS se correlacionam positivamente em pacientes obesos.

Introduction: The obstructive sleep apnea (OSA) is caused by recurrent episodes of partial or total obstruction of the upper airway lasting more than 10 seconds during sleep. Laryngopharyngeal reflux (LPR) is a variant of the disease Gastroesophageal Reflux that affects the larynx and pharynx. Objectives: Evaluate the influence of obesity on the relationship between RFL and OSAS in patients with OSA. Materials and methods: An observational retrospective cross. We reviewed care protocol for patients with OSA that includes validated questionnaires for RFL as Sympton Reflux Index (RSI) and Reflux Finding Score (RSI), and polysomnography nasolaringofibroscopia. Results: 105 patients were divided into obese group (39 patients) and non-obese patients (66 patients). In the evaluation of the mean RSI group of non-obese was similar between patients with mild OSA (11.96) and moderate (11.43). In the obese group the mean RSI was 6.7 in patients with mild OSA and 11.53 in patients with moderate to severe OSA (p < 0.05). Discussion: The subgroup of patients with OSA and RFL have several factors that promote inflammation of the upper airway. Patients with OSA should be screened and treated as the RFL increasing the quality of life. Conclusion: The RFL are positively correlated and OSAS in obese patients.

PURPOSE: To develop an experimental model to study and radiologically monitor displacement of skin flaps in the pericranium of rats subjected to traction and surgical fixation using suture anchored in a skull bone tunnel or with N-butyl-2-cyanoacrylate (HistoacrylTM) surgical adhesive. METHODS: Radiological markers were placed in the subcutis of Wistar rats undergoing subperiosteal detachment of the pericranium with pulling and fixation of the flap. We performed radiography on postoperative days 3, 7, 14, 21, and 45. A p-value of <0.05 was considered significant. RESULTS: Qualitative analysis of the data indicated that the flaps in the surgical adhesive group remained in place with no change from the immediate postoperative position. However, the flaps in the suture anchored in the skull bone tunnel group and in the control group showed similar healing, with a loss of attachment of 9.7% and 22%, respectively, compared with the immediate postoperative position. There was no quantitative difference between the groups. CONCLUSIONS: This experimental model created acceptable experimental conditions for testing different soft tissue fixation methods. The use of tissue fixatives contributed to better placement of pericranium-cutaneous flaps, and surgical adhesive was superior to suture anchor in the skull bone tunnel for fixation of pericranium-cutaneous flaps.

INTRODUÇÃO: As fraturas nasais são lesões de grande incidência, frequentemente consideradas de menor importância. No entanto, podem trazer prejuízos importantes do ponto de vista tanto funcional como estético. MÉTODO: Foram analisados prontuários médicos de 144 pacientes submetidos a tratamento de fratura nasal na Área de Cirurgia Plástica do Hospital de Clínicas da Unicamp, no período de fevereiro de 2002 a outubro de 2008. RESULTADOS: Observou-se predomínio de pacientes do gênero masculino (75,7%), com proporção entre gêneros masculino e feminino de 3,1:1. A faixa etária mais acometida foi aquela entre 21 anos e 30 anos de idade. A maioria das fraturas (31,8%) foi causada por agressão física. Todos os pacientes foram tratados por meio de redução fechada, sob anestesia local e tópica, após um período médio de 8 dias. Entre os pacientes analisados, 13 evoluíram com deformidade residual. CONCLUSÕES: O paciente tipicamente afetado pelas fraturas nasais é o indivíduo adulto jovem e do gênero masculino. O tratamento cirúrgico por meio de redução fechada promove resultados aceitáveis, se respeitados os princípios de avaliação correta da lesão e do momento de indicação do tratamento.

INTRODUCTION: Nasal fractures are common injuries, frequently considered to be minor. Nevertheless, they may cause significant damage, from both a functional and aesthetic perspective. METHODS: Medical records of 144 patients admitted for nasal fracture treatment between February 2002 and October 2008 to the Plastic Surgery Area of the Unicamp Clinical Hospital were analyzed. RESULTS: Patients were predominantly male (75.7%), with a male to female ratio of 3.1:1. The most number of cases was observed in 21 to 30 year olds, and the majority of fractures (31.8%) were caused by physical aggression. Closed reductions were performed on all patients under local and topical anesthesia after an average of 8 days. Among the studied patients, 31 experienced residual deformity. CONCLUSIONS: Patients typically affected by nasal fractures are young male adults. Closed reduction surgical treatment yields acceptable results, with correct injury evaluation and treatment timing.

Os bisfosfonatos são potentes inibidores da reabsorção óssea e são utilizados no tratamento da osteoporose e de outras doenças que causam a perda de massa óssea, como doença de Paget, metástases ósseas e mieloma múltiplo, prevenindo fraturas patológicas. Desde 2003, estudos associam osteonecrose avascular dos ossos maxilares a seu uso, principalmente intravenoso. Na literatura, há relatos de ocorrência variando de 0,8% a 12%, dos pacientes, na sua maioria em uso prolongado. Médicos e dentistas devem estar cientes dessa potencial complicação no tratamento odontológico.

Bisphosphonates are potent inhibitors of bone resorption, and are used in the treatment of osteoporosis and other diseases that cause bone mass loss, such as Paget's disease, bone metastases, and multiple myeloma, to prevent pathological fractures. Since 2003, avascular osteonecrosis of the jaw has been associated with the use of bisphosphonates, mainly intravenous. According to the literature, the occurrence of osteonecrosis of the jaw has ranged from 0.8% to 12% of the patients on bisphosphonates, most of them on prolonged use. Physicians and odontologists should be aware of that potential complication in dental treatment.

O tratamento de lesões traumáticas da órbita permanece um desafio para o cirurgião maxilofacial. Quando a correção cirúrgica não é realizada ou é feita de maneira inadequada, pode ocorrer enoftalmia, diplopia, distopia ocular, restrição da movimentação ocular e disfunção do nervo infraorbital. A importância da cirurgia consiste em liberar o tecido orbitário herniado pelo foco de fratura, restaurar a arquitetura normal da órbita, objetivando um resultado estético e funcional adequado. Nas últimas décadas, vários avanços ocorreram no tratamento cirúrgico destas fraturas, bem como nos métodos diagnósticos. Com o desenvolvimento de tomografias computadorizadas de múltiplos detectores, tornou-se possível a análise tridimensional da órbita, assim como sua avaliação volumétrica, o que revolucionou o manejo cirúrgico destas fraturas. Outro fator com impacto direto nas reconstruções das órbitas é a disponibilidade de diversos biomateriais, para restauração das paredes orbitárias. Assim, o objetivo desse artigo é revisar os materiais disponíveis para reconstrução nos casos de fraturas do assoalho da órbita, comparar suas aplicabilidades práticas e destacar aqueles mais utilizados no Serviço de Cirurgia Plástica do Hospital de Clínicas da Unicamp, nos últimos anos. Dentre os materiais de escolha, em nosso Serviço, destacamos o osso autógeno de calota craniana, a cartilagem de concha auricular, a tela de titânio e o polietileno poroso de alta densidade. Cada biomaterial apresenta indicações específicas, de acordo com as características da fratura orbitária, sendo considerados, para a escolha do material, os resultados a longo prazo e a experiência do cirurgião.

The treatment of traumatic lesions of the orbit remains a challenge for the maxillofacial surgeon. When surgical correction is not performed or is performed incorrectly, may occur enophthalmos, diplopia, ocular dystopia, restriction of ocular movements and dysfunction of the infraorbital nerve. The importance of surgery is to release the herniated orbital tissue at the fracture site, restoring the normal architecture of the orbit, aiming at an adequate functional and aesthetic results. In recent decades, many advances have occurred in the surgical treatment of these fractures, as well as in diagnostic methods. With the development of multislice CT, it became possible to analyze three-dimensional orbit, as well as its volumetric evaluation, which revolutionized the surgical management of these fractures. Another factor with direct impact on the reconstruction of the orbits, is the availability of various biomaterials for the restoration of the orbital walls. The objective of this paper is to review the materials available for reconstruction in cases of fractures of the orbital floor, its applicability to compare practices and highlight those most used in the Plastic Surgery Service, Hospital de Clinicas, Unicamp, in recent years. Among the materials of choice in our service, we highlight the autogenous skull bone, cartilage of ear shell, the titanium mesh and porous high density polyethylene. Each biomaterial has specific indications, according to the characteristics of orbital fracture, being considered for choosing the material, the long-term results and experience of surgeon.

OBJECTIVE: The purpose of this study was to perform a clinical retrospective analysis of the etiology, incidence and treatment of selected oral and maxillofacial injuries in Brazilian children and adolescents. MATERIAL AND METHODS: This study was conducted during a 14-year period between 1986 and 2000. All patients were admitted to Hospital XV in the city of Curitiba, State of Paraná. Age, gender, monthly distribution, etiology, soft injuries, associated injuries, site of fractures and methods of treatment were reviewed. RESULTS: Of the total of 350 patients of all ages treated for facial injuries, 29.42% were within the age range of the study (0 to 18 years). Mean age was 10.61. Of the patients, 63.1% were male. The most common cause of injury was accidental falls (37.87%), followed by bicycle and motorcycle accidents (21.36%). Of the 103 patients, 88.34% had single injuries. Mandibular fractures were the most common and the condylar region was particularly affected. CONCLUSIONS: Facial trauma is a relatively common occurrence in children. The study indicates that fractures in children and adolescents differ quite considerably from an adult population.

Este estudo cefalométrico retrospectivo foi proposto objetivando analisar a estabilidade do reposicionamento anterior da maxila, utilizando-se a osteotomia Le Fort I. A amostra consistiu de 30 telerradiografias de dez pacientes submetidos a um avanço cirúrgico da maxila, sem segmentação desta ou associação com qualquer cirurgia na mandíbula, realizado pela Área de Cirurgia Buco-Maxilo-Facial da Faculdade de Odontologia de Piracicaba - Unicamp. A mesma técnica de fixação interna rígida foi utilizada para todos os pacientes, e nenhum enxerto ou substituto ósseo foi usado. Três radiografias cefalométricas laterais foram realizadas para cada paciente, nos períodos pré e pós-operatórios imediatos, e com, no mínimo, seis meses após a cirurgia. Sobre estas radiografias foram feitas linhas de referência horizontal e vertical. Os pontos cefalométricos utilizados para calcular as mudanças de posição da maxila foram tanto esqueléticos, quanto dentários. A análise estatística (teste t de student pareado) não demonstrou diferença estatística significante entre os intervalos pós-operatórios imediato e com no mínimo seis meses. Concluímos que o reposicionamento anterior da maxila, utilizando-se a osteotomia Le Fort I, com fixação interna rígida e sem o uso de enxertos autógenos ou qualquer substituto ósseo, é um procedimento estável.

This is a retrospective cephalometric study designed to investigate the stability after anterior maxilla repositioning, by Le Fort I osteotomy. Thirty cephalometric radiographs were selected, from ten patients submitted to one-piece surgical maxillary advancement, with no concomitant mandibular surgery, performed by the Division of Oral and Maxillofacial Surgery of Piracicaba Dental School - Unicamp. These cases were submitted to the same internal rigid fixation technique, with no use of bone grafts or any bone substitute. Three lateral cephalometric radiographs were obtained for each patient, immediately presurgery and postsurgery, and at least six months after surgery. Lines were draw upon the X-rays to serve as horizontal and vertical reference lines.The landmarks used to measure maxillary positional changes consisted of skeletal and dental features. Statistical analysis (paired t student test) showed no significant differences between the immediately postsurgery and at least six months postsurgery periods. We concluded that the anterior maxillary repositioning by Le Fort I osteotomy, using rigid internal fixation, with no bone grafts or any bone substitute, is a stable procedure.

Actualmente la reducción abierta de las fracturas mandibulares y la fijación y estabilización de los segmentos óseos por medio de la fijación interna rígida es considerado el tratamiento de elección versus técnicas mas conservadores como lo son la reducción cerrada y el bloqueo maxilo-mandibular o el uso de osteosíntesis por medio de alambre de acero. La fijación interna rígida es una técnica que brinda la suficiente estabilidad para propiciar un correcto reparo óseo así como brindar al paciente un postoperatorio mas confortable y una reincorporación mas temprana a su vida normal, sin limitar notablemente sus actividades fisiológicas y sociales, en comparación con las técnicas antes mencionadas.

At the moment, the open reduction of the mandibular fractures and the fixation and stabilization of the bone segments by rigid internal fixation is considered the election treatment versus more conservative techniques as are the closed reduction and the intermaxillary fixation or the use of osteosíntesis by steel wire. The rigid internal fixation is a technique that offers enough stability to propitiate a correct bone repair as well as to offer the patient a more comfortable postoperative period and an early reincorporation to a normal life, without limiting his activities, in comparison with the techniques before mentioned.

Atualmente, a redução aberta das fraturas mandibulares e a fixação dos segmentos ósseos por meio da fixação interna rígida é considerado o tratamento de eleição frente a técnicas mais conservadoras como são a redução fechada e o bloqueio maxilo-mandibular, ou o uso de osteosíntesis por meio de fio de aço. A fixação interna rígida é uma técnica que oferece a suficiente estabilidade para propiciar um adequado reparo ósseo, além de oferecer ao paciente um pós-operatório mais confortável e uma reincorporação mais precoce a suas atividades, sem limitar notavelmente suas atividades fisiológicas e sociais, em comparação com as outras técnicas anteriormente mencionadas.
